Home Health Care

Our nurses, physical therapists, occupational therapists, and licensed nursing assistants provide home care for patients recovering from hospitalization or illness. Speech therapy and medical social worker services available through contractual agreement.

Enhanced Speciality Program for Home Care Clients with a Diagnosis of Congestive Heart Failure (CHF) 

This program involves the use of a ZOE® machine to measure chest fluid, client education tools, and assessments performed by a registered nurse trained in cardiac care.  The goal of the program is to improve client outcomes, reduce re-hospitalizations and improve quality of life.

Support Programs

NANA offers Senior Companions as part of the State and Federally funded program.

Clinics

Free Blood Pressure, Foot Care and Well Child Clinics and low-cost Flu Shot Clinics are held on a scheduled basis.  Please contact NANA at 603-744-2733 to learn more or to schedule an appointment.
